- Please read these instructions before filling out the form. You may wish to have them open in another window while completing the form. NOTE: if you apply for the course after the June 2, 2008 deadline, it is better to not send in the extra paperwork until requested. In that case, we would only request any extra paperwork from those who are accepted.
- Additional Form Required If You Must Pay
- If you must pay a fee to attend the course (see the fee schedule), you must also fill in this special form and return it, signed, not later than May 10, 2008. Send the form to the address given on the form. Exception: Paying KTH students (Ph.D.s) do not need to fill in this form; paying SU students should fill in the form.
- Additional Form for Paying Ph.D. students from KTH
- Paying PhD students from KTH shall consult the following web page for additional details, requirements, forms to fill, etc. This information is only available in Swedish.
- Additional Paperwork Required If No Previous Account
- After registering for the course, there is some additional paperwork you need to send to PDC if you do not have a PDC or CSC (NADA) computer account already. Get the details here.

- Fill in this form if your fee is SEK 1800 (i.e., normally SEK 1700, with a SEK 100 late fee)
